What information should be included in a substance identity adaptation request?

注册人需要集体提交其物质身份变更申请，并提交“联合提交计划”模板（可通过专用网络表单获取）中包含的信息。该模板列出了服务请求所需的最低限度信息，可用于联合提交和单独提交。务必确保申请中附带的“联合提交计划”完整一致，并且申请中包含所有必要文件（包括书面证据），以证明所有申请变更的注册人：(i) 同意变更，以及 (ii) 承诺在 ECHA 规定的期限内更新其注册档案。

Registrants will need to collectively submit with their substance identity adaptation request the information included in the ‘Joint Submission Plan’ template (available via the dedicated webform). The template lists the minimum information needed for the service request and can be used both for joint and individual submissions.  It is important that the ‘Joint Submission Plan’ attached to the request is complete and consistent, and that the request includes all required documentation (including documentary evidence) that all registrants who request a change: (i) agree with the changes, and (ii) commit to update their registration dossiers by the timeline set by ECHA.
